SCS Webform (Jotform) Outage

DETAILS:
Some users of our SCS Webforms system, Jotform, are experiencing intermittent issues viewing forms. We have reported the outage to our vendor and are awaiting updated information and outage resolution. We will resolve this alert once the system is back online and reliably available.
